The case manager is a key member of the multidisciplinary H2H team, working in hospital, office, residential, and community settings to support individuals experiencing homelessness or housing instability with significant mental health, behavioral health, and medical needs. This role prioritizes candidates with experience in mental health care and crisis intervention, particularly in hospital or acute-care environments.The Case Manager provides intensive, person-centered, strengths-based case management services to support individuals transitioning from hospital care to stable housing.
