Role Description The Field Nurse Case Manager: Greater Kansas City role is a full-time, on-site position based in Kansas City, MO. The professional in this role will conduct in-person assessments, review medical histories, and evaluate current health status to identify needs and risks. Daily responsibilities include developing and updating individualized care plans, coordinating services with physicians, therapists, and other providers, and facilitating access to appropriate medical and community resources. The Field Nurse Case Manager will monitor progress toward clinical and functional goals, document encounters and care plan changes, and advocate for clients to ensure timely, evidence-based interventions. This role also includes educating clients and families on diagnoses, treatments, and self-management strategies, while maintaining compliance with regulatory and organizational standards.
Qualifications

• Clinical expertise in nursing and case management, including experience in patient assessment, care planning, and care coordination.
• Strong skills in communication, education, and advocacy to effectively engage clients, families, and interdisciplinary healthcare teams.
• Proficiency in documentation, electronic health records, and basic computer applications to maintain accurate, timely case records.
• Ability to work independently in the field, manage a caseload, prioritize tasks, and adapt to changing clinical needs.
• Current Registered Nurse (RN) license in the state of Missouri (or eligibility for licensure) and relevant clinical experience in acute, rehab, or community settings.
• Knowledge of community resources, insurance processes, and healthcare regulations related to case management.
• Valid driver’s license, reliable transportation for on-site visits, and willingness to travel within the Greater Kansas City area.
• Certification in case management (e.g., CCM) or willingness to obtain certification is preferred.
