Responsibilities

  • Deliver direct nursing care to residents following individualised care plans and facility policies.
  • Administer medications and treatments safely within LPN scope of practice.
  • Monitor, document, and promptly report changes in resident condition to the supervising RN and provider.
  • Collaborate with CNAs and the interdisciplinary team to support resident goals and care plan compliance.

Requirements

  • Active, unencumbered LPN licence in the applicable state.
  • Minimum 1 year SNF, LTC, or post-acute experience preferred.
  • Current BLS/CPR certification.
  • Reliable, detail-oriented, and committed to compassionate resident care.
