About WTC Support Services
WTC Support Services provides person-centered Coordinated Family Support (CFS) services to individuals with intellectual and developmental disabilities and their families. We are seeking compassionate, organized, and motivated professionals who are committed to empowering families, promoting independence, and delivering exceptional service.
Position Summary
The Support Coordinator is responsible for managing a caseload of individuals receiving Coordinated Family Support (CFS) services through the Regional Center. This role involves conducting assessments, developing individualized support plans, coordinating resources, advocating for families, maintaining timely documentation, and collaborating with community partners to help participants achieve their goals.
Essential Responsibilities
  • Conduct intake assessments and develop individualized service plans.
  • Manage and monitor assigned caseloads.
  • Coordinate services with Regional Centers, healthcare providers, schools, and community agencies.
  • Assist families in accessing resources such as IHSS, SSI, Medi-Cal, CalFresh, transportation, employment, housing, respite, day programs, and recreational services.
  • Advocate for participants and identify solutions to barriers impacting services.
  • Provide education, guidance, and ongoing support to individuals and caregivers.
  • Participate in IPP meetings and interdisciplinary team meetings.
  • Maintain accurate, timely documentation, including progress notes, assessments, monthly summaries, quarterly reports, and communication logs.
  • Meet agency productivity, documentation, and compliance standards.
  • Attend required trainings and staff meetings.
Qualifications
Preferred Education
Bachelor's degree in Social Work, Psychology, Human Services, Sociology, Nursing, Special Education, Child Development, or a related field. Equivalent experience may be considered.
Preferred Experience

  • At least one year of experience in case management, care coordination, developmental disabilities, behavioral health, social services, or Regional Center programs.
  • Knowledge of the California Regional Center system, Lanterman Act, and person-centered planning is highly preferred.
Skills & Requirements
  • Excellent communication, organization, and time management skills.
  • Strong critical thinking and problem-solving abilities.
  • Ability to work independently while collaborating with a multidisciplinary team.
  • Proficient in Microsoft Office and electronic documentation systems.
  • Valid California driver's license, reliable transportation, and current auto insurance.
  • Ability to travel within assigned service areas.
  • Pass DOJ/FBI Live Scan background clearance.
  • CPR/First Aid certification (or willingness to obtain).
