Description:

Position Summary:

The Direct Support Caregiver plays a key role in supporting the design, coordination, and delivery of day-to-day services for individuals with disabilities in a site-based setting. This position ensures that programming is person-centered, promotes independence, and helps participants work toward their individual goals. The caregiver works closely with staff, families, and community partners to maintain a safe, supportive, and enriching environment for all participants.


Key Responsibilities:

  • Develop and implement individualized service plans in coordination with clients, caregivers, and interdisciplinary teams.
  • Facilitate skill-building activities, group sessions, and structured programs focused on independence, communication, community integration, and health.
  • Monitor and document client progress; make updates to plans as needed based on observations, outcomes, and feedback
  • Ensure all programming and activities follow licensing standards, organizational policies, and health and safety guidelines.
  • Provide direct support as needed, including assistance with mobility, self-care, or behavior support in accordance with individual support plans.
  • Maintain accurate, timely, and confidential client records and documentation.
  • Collaborate with case managers, therapists, job coaches, and other support staff to ensure coordinated and consistent care.
  • Participate in team meetings, interdisciplinary reviews, and ongoing professional development.
  • Support the planning and execution of site events, community outings, and partnership activities.
Requirements:
  • High school diploma or equivalent required; associateโ€™s or bachelorโ€™s degree in human services, education, psychology, or a related field preferred.
  • Minimum of 1โ€“2 years of experience working with individuals with disabilities or in a similar support role.
  • Strong communication, organization, and interpersonal skills.
  • Ability to handle crises calmly and professionally.
  • Basic computer skills for documentation and reporting.
  • Must pass background checks and any required certifications (e.g., CPR/First Aid, crisis intervention).
  • Physical ability to assist clients as needed, including lifting or supporting mobility aids.

Preferred Attributes:

  • Knowledge of person-centered planning and disability rights.
  • Bilingual (Spanish/English or ASL) a plus.
  • Familiarity with Medicaid waiver programs, DARS/TWC, or local disability service systems.
