Case Manager Supervisor - Hennepin County Waiver Case Management

People Incorporated Mental Health Services is seeking a Case Manager Supervisor to oversee our Hennepin County Waiver Case Management team. This position supervises a team of six Case Managers and plays a key role in ensuring clients receive high-quality, person-centered services that meet county, state, and organizational standards.

Our Waiver Case Management team supports individuals living in the community by coordinating Medicaid-funded services that promote health, independence, and quality of life. Case Managers build meaningful, long-term relationships with the people they serve through regular contact, comprehensive service planning, and ongoing advocacy and support.

People Incorporated values flexibility, autonomy, and work-life balance. After completing initial training, Case Managers manage their own hybrid schedules while receiving ongoing support from leadership. Supervisors work in the office two days per week to collaborate with colleagues, mentor staff, and foster team engagement. Our manageable caseloads average approximately 30 individuals per Case Manager, allowing staff to provide thoughtful, responsive services while maintaining a healthy work-life balance.

Hours: Full-time, Monday through Friday, regular business hours

Location: Hennepin County, Minnesota | Hybrid

Hiring Range: $64,000 - $68,000 Salary, based on experience and qualifications, plus $1,000 to external candidates

Main Job Duties

  • Supervise and coach assigned program staff to ensure adherence to quality standards, organizational policies, and established procedures
  • Provide regular 1:1 supervision, performance feedback, and support to promote high-quality service delivery
  • Recruit, hire, orient, and retain high-quality staff while fostering a positive, inclusive, and supportive team culture
  • Assess employee strengths and development needs, provide training and mentorship, and offer timely, specific feedback related to job performance, best practices, and company expectations
  • Partner with staff to identify and achieve professional development goals that support growth within their current roles and future career advancement opportunities
  • Coordinate program schedules, staff meetings, and coverage plans to ensure continuity of services and effective team communication. Co-facilitate meetings and projects as needed
  • Provide direct client services in an integrative, collaborative, and person-centered manner when appropriate
  • Model person-centered practices and support staff in delivering services that promote client choice, independence, and quality of life
  • Facilitate partnerships and maintain strong working relationships with clients, families, providers, counties, and other stakeholders to support positive outcomes and address the overall needs of individuals served
  • Create and maintain a strong sense of community, collaboration, and engagement among staff and clients
  • Develop and maintain effective working relationships with program staff, organizational leadership, and other program managers to support collaboration, problem-solving, and program success
  • Maintain a regular physical presence within the Twin Cities metropolitan area and participate in in-person supervision, meetings, and program activities as needed.

  • Passing all background and requirements for facility and position; and
  • The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
  • Must have a valid and current driver’s license and auto insurance

Required Education and Experience:

  • Bachelor’s Degree in the behavioral sciences or related field (i.e. social work, psychology, or nursing from an accredited college or university).
  • One year or 2,000 hours of prior experience in the human services field with a preference towards servicing those with mental health symptoms, or completion of People Incorporated’s Link to Leadership program
  • Two years' experience as a Waiver Case Manager required

Preferred Education and Experience:

  • Two years prior supervisory experience in the delivery of behavioral health services.
  • Completion of the People Incorporated’s Link to Leadership Program.
  • Master’s degree in a related field.
  • Candidates who hold an active and unrestricted license in the State of Minnesota (LAMFT, LGSW, LADC, LSW or LPC)
