Job description

Scope:                        To ensure that each client within the assigned caseload is sufficiently supported by the Skyland Trail program services from the client's entry into the program until discharge.  Responsible for well-planned and coordinated therapeutic services within the CBT and DBT recovery program. To serve clients from various tracks according to evidence based treatment modality to fit diagnosis.

Duties:                        Coordinates all aspects of services to support the goals of client (i.e. recovery plan meetings, treatment planning, and discharge planning).

Develops individualized recovery plans (complete with day treatment schedule) for caseload and revises as necessary with input of recovery team.  Conducts individual sessions at least once per week with assigned caseload, also reviews clients’ treatment plans and documents progression towards recovery plan goals.  Records all changes in status as necessary.

Oversees daily core groups and assures coverage for assigned recovery track. Leads weekly recovery team meetings and makes professional case presentation.  Communicates recovery goals, wellness plans, expectations and opportunities for clients. 

Participates in group supervision bi-weekly.  Participates in weekly supervision with CBT Track Assistant Director for administrative purposes, support and case consultation.

Works with recovery team to design wellness plans.  Set appropriate wellness plans for caseload as needed, and reports plan to the client, family and clinical staff.

Completes all documentation in accordance with agency guidelines.  Complete progress report on each client and completes concurrent reviews for UR staff.  Conducts insurance reviews as needed for caseload. Maintains contact with insurance companies as required by each managed care organization’s standards.

Maintains verbal or written contact with families at a minimum of once every 2 weeks, and has family sessions at a minimum of once every month.

Practices ethically and complies with facility policies and accreditation standards (i.e., Joint Commission, ORS, etc.)

Assists and guides clients to achieve the maximum level of independence, while functioning in the least restrictive environment.

Refers clients to appropriate services, internally and externally. Maintains contact with other service providers.

Conduct assigned Day Treatment groups and document in electronic health record.  Develop written curriculums for each group. Utilize curriculum resources to plan, facilitate and/or co-facilitate assigned day treatment groups with the assistance of the Day Services Director.

Supervise master’s level intern(s) weekly as needed and have regular contact with school advisor.

Provides transportation for clients as needed. 

                                    Participate in continuing education program in specialty area.

Complete weekly to bi-weekly assessments for monitoring symptoms with caseload.

Attends CBT and DBT consultations groups as scheduled.

Completes on-call rotation for 4 weeks out of the year, including one holiday.

                                    Completes other duties as assigned.  

  • Master degree in related field (i.e.; Psychology, Social Work, Counseling, etc.).
  • Prior training/experience with CBT and/or DBT group and/or individual counseling.
  • Prior experience in partial hospitalization/day treatment setting for chronically mentally ill.
  • License preferred.
