Position Summary:

The CTI Case Manager provides a nurturing presence, offering active listening, support, and guidance to families and young people facing life challenges in our community. This position ensures that households receive the skills, knowledge, and resources necessary to achieve stability and thrive. This position will involve collaboratively assessing needs, creating personalized support plans, and providing case management services. The CTI Case Manager must demonstrate exceptional empathy, strong interpersonal skills, and a commitment to social justice, ensuring that every individual is treated with dignity and respect.

Specialty: Critical Time Intervention (CTI) Case Management

Critical Time Intervention (CTI) Case Managers focus on supporting families and youth during the critical period after exiting homelessness, helping them stabilize, stay housed, and avoid returning to homelessness. They provide short-term, intensive support, strengthen natural and community connections, and address barriers that may threaten housing stability. CTI Case Managers are also cross-trained as Housing Navigators, allowing them to assist with ongoing housing needs and help participants maintain long-term stability.

Essential Duties & Responsibilities:

• Develop meaningful, trusting relationships with participants to foster a safe and supportive environment for open communication and healing.
• Conduct thorough needs assessments for individuals and families facing challenges related to poverty, homelessness, housing instability, and trauma. Develop individualized support plans that emphasize skill-building and resource connectivity.
• Provide ongoing case management services, including regular check-ins, monitoring progress, and adjusting support plans as necessary to meet changing participant needs.
• Provide case management in the participants home, within the community, or in any environment where they feel most comfortablemeeting them where they are, both physically and emotionally.
• Assist participants in navigating available community resources and services, including housing, employment, education, mental health, and addiction support ,etc.
• Act as an advocate for participants, helping them to understand their rights and navigate systems that may be overwhelming or challenging.
• Facilitate one-on-one sessions focusing on essential life skills, financial literacy, job readiness, and emotional regulation techniques, etc.
• Work collaboratively with social service agencies, healthcare providers, and community organizations to enhance service delivery and improve outcomes for participants.
• Maintain accurate and confidential case records and documentation in compliance with organizational policies and reporting requirements.
• Engage in community outreach efforts to educate the public about available services and promote a culture of compassion and support.
• Other duties as assigned.

General Expectations and Responsibilities:

• Collaborate with community agencies, members, and organizations to become familiar with the services they offer.
• Maintain a professional and respectful disposition at all times.
• Deliver all services with a high level of customer focus and service in accordance with training, NPP policies and procedures, program requirements, and direction from leadership.
• Engage in active listening and use clear communication and verbal skills.
• Document all pertinent data and information regarding work with individuals, agencies, and other community members promptly in accordance with company requirements.
• Participate in organization planning.
• Attend staff meetings and trainings
• Attend community meetings as directed.
• Update shared calendar to reflect activities, meetings, and general schedule.

Supervisory Responsibilities:

The CTI Case Manager requires no supervisory responsibility.

Qualifications:

Car Use:

The position will require operating a company vehicle and/or personal vehicle to transport goods or personnel to various locations as needed. A valid HI driver's license and a clean driving record are required for this role. A reliable personal vehicle is required for this role.

Education and/or Experience:

Preferred: Associate Degree or higher

Required: High School Diploma or equivalent and 1 year of experience serving vulnerable and challenged individuals.

Computer Skills:

To perform this job successfully, an individual must have knowledge of basic office software especially: MS Word or Google Docs, MS Excel or google sheets, and generally be familiar and comfortable using computers and various programs and apps.

Skills and Knowledge:

• Excellent computer literacy and knowledge of Microsoft Suite, Google Docs, and Google Sheets.
• Ability to communicate effectively and courteously with families, staff, and community members.
• Knowledge of available community resources.
• Ability to handle multiple tasks and adhere to deadlines.
• Proven organizational and planning skills.
• Ability to work as part of a team and maintain flexibility around task assignments.
• Ability to establish, recognize, and adhere to appropriate professional boundaries.
• Proven in providing direct service to families and/or individuals.

Other Requirements:

• Valid Hawaii drivers license and no-fault car insurance.
• Clean driving abstract.
• Resident of Puna/East Hawaii Community area preferred.
• Criminal Record and/or DHS-CWS background check
• Participate in the upkeep of office and general cleanliness of workspace

It is not uncommon to see, engage, or be confronted with the following: violence and threats of violence; profane, racists and/or sexist language; bodily fluids; conflict; interactions with First Responders; alcohol and other street drugs; cigarette smoke; death of service participants; nudity of service participants; people involved with sex work; people involved in the drug trade; persons used against their consent, will or knowledge; people in conflict with the law; and/or other situations that may be unsettling. Measures are taken to train staff to deal with these situations appropriately.
