Job description

Job Title: Primary Care Physician
Location: Chicago, IL
Compensation: Base salary starts at $250,000 with an incentive bonus up to $20,000 in the first year (and growing as a partner). Signing bonus up to $50,000 or a $100,000 forgivable loan. Clear path to partnership within 12-18 months - no buy-in required. In fact, 65% of physicians make partner within the first year, with total annual compensation typically over $300,000 once partnered.
We are a physician-led, mission-driven primary care group delivering high-quality, compassionate care to seniors with complex health needs. Our mission is to honor seniors with affordable, VIP-level primary care that truly improves health outcomes and quality of life. We believe doctors should have the time, resources, and support they need to care for patients properly - with no assembly-line medicine. For patients who need it, we even provide free transportation to and from the clinic, so no one is left behind.
As a Primary Care Physician with us, you'll enjoy a full-time, 100% outpatient practice. Expect a Monday through Friday schedule with no nights or weekends. Clinic hours run from 7:30 AM to 5:00 PM, with your last patient scheduled at 4:15 PM. You'll see an average of just 18 patients a day and maintain a small patient panel of no more than 450 seniors, typically managing 3-5 chronic conditions. You'll have dedicated admin time built into your day and a care team that includes your own medical assistant, scheduler, and referral specialist - so you can focus on practicing real, value-based care instead of rushing through visits.
This role comes with generous time off and excellent benefits. You'll receive 33 days of paid time off, a full benefits package for you and your family (health, dental, vision, and supplemental plans), a 401(k) with 5.5% match, plus $3,500 annually for CME and an extra week to use it. On-site, you'll also have access to in-house consulting specialists including cardiology and podiatry, along with holistic health services like acupuncture, Tai Chi, yoga, and nutrition. For patients, free door-to-door transportation means better access and engagement - and less worry for you about no-shows.
To qualify, you must be Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine. You'll need an active or clearable license in Pennsylvania and a current DEA.
