As a Home Health Registered Nurse, you'll provide skilled, personalized care that helps patients recover from illness, manage chronic conditions, avoid unnecessary hospitalizations, and remain safely in the comfort of their own homes.

What You'll Do
  • Perform comprehensive nursing assessments and skilled visits
  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
  • Develop and implement individualized plans of care
  • Provide skilled nursing services, including medication management, wound care, disease management, IV therapy as appropriate, and patient education
  • Monitor patient progress and communicate changes with physicians and the interdisciplinary care team
  • Coordinate care with therapists, occupational therapists, speech therapists, social workers, and home health aides
  • Complete timely and accurate clinical documentation using WellSky
  • Educate patients and caregivers to promote safety, independence, and improved health outcomes
Qualifications
  • Active Registered Nurse (RN) License in Washington, DC, Maryland, or Virginia required
  • Multi-state licensure or eligibility to practice in multiple jurisdictions within the DMV is preferred
  • Valid Driver's License and Reliable Transportation
  • Strong Clinical Assessment, Critical Thinking, and Documentation Skills
  • Home Health Experience Preferred but Not Required
  • OASIS Experience Preferred
  • Experience with WellSky Home Health is a plus
