CALLING ALL CERTIFIED HOME HEALTH AIDES!

We are privately owned CHAP Accredited Home Health Agency, based in Campbell, California. We currently provide home health care to patients in Santa Clara county, San Mateo County and San Benito county, and are expanding into Alameda, Contra Costa & Solano counties.

Ascension Healthcare has an immediate need for a Certified Home Health AIDE in Santa Clara County (San Jose to Palo Alto and cities in between ) and San Benito County (Morgan Hill, Gilroy, Hollister, San Juan Bautista and surrounding areas)

Our founders include 150+ years of highly experienced nurses, healthcare administrators and other professionals in the post-acute industry dedicated to the highest quality clinical outcomes and patient satisfaction.

Compensation - DOE with Oasis Home Health

Responsibilities:

- Provide compassionate and personalized care to patients in their homes

- Assist with activities of daily living, including bathing, dressing, and grooming

- Administer medications as prescribed by healthcare professionals

- Monitor and document patient vital signs and report any changes or concerns to the appropriate healthcare provider

- Assist with mobility and transfers, ensuring patient safety at all times

- Provide companionship and emotional support to patients and their families

- Assist with meal planning, preparation, and feeding as needed

- Follow care plans and instructions provided by healthcare professionals

- Maintain a clean and safe environment for patients

Qualifications:

- Previous experience in home health or a related field is preferred

- Knowledge of hospice care, dementia care, assisted living, and memory care is a plus

- Ability to administer medications accurately and safely

- Familiarity with tube feeding systems is desired- Strong communication skills, both verbal and written

- Knowledge of HIPAA regulations and ability to maintain patient confidentiality

- Compassionate and empathetic nature, with a genuine desire to help others

- Ability to work independently and as part of a team
