Job Summary:
Clinical Duties Include:
  • Rendering outpatient primary care medical and consulting services for ArchWell Health, pursuant to one or more physician supervision and collaboration agreements as required by law and/or ArchWell Health
  • Keeping and maintaining (or causing to be kept and maintained) appropriate records of all professional services
  • Appropriately documenting the care of patients which includes ensuring the accuracy of patient records, encounter notes, medical diagnoses, and documentation
  • Working with care team members and other employees in a collaborative manner consistent with ArchWell Health's policy of providing high quality care while maintaining excellent morale among colleagues
  • Working with colleagues to ensure a high-quality patient experience, that minimizes wait times and allows patients with urgent needs to be evaluated in a timely manner
  • Being available to participate in public relations, direct sales, or outreach events inside or outside of ArchWell Health centers in order to engage the community and promote the ArchWell Health model of care
  • Participating in clinical meetings which include reviews of hospitalized patients, clinical outcomes, quality improvement and/or peer review activities
  • Being available for call coverage on nights, weekends and/or holidays with respect to patients of the center(s) where the provider regularly provides clinical services or, if requested, to patients of other ArchWell Health centers, pursuant to a call schedule determined by ArchWell Health in its discretion, provided the provider's obligations will be consistent with the demands placed on other providers

Acute Care Nurse Practitioner Clinical Duties:
  • Render primary care and consulting services for ArchWell Health, with a focus on acute care needs, including but not limited to walk-in appointments, sick visits, hospital follow-ups, and other acute needs
  • Support and partner with RNs and other clinical and operations colleagues to provide appropriate, timely care to members
  • Communicate and document plan of care with members' Primary Care Provider in order to maintain strong documentation and continuity of care
  • Partner with operations leadership and medical leadership to identify members in need of acute support, outreach to members, and perform appropriate services in alignment with ArchWell Health policies and procedures
  • Partner with Medical Assistants and other members of the ArchWell Health care team and colleagues to provide high quality care

Education/Experience:
  • Currently licensed to practice professional nursing in the state of employment, active and non-probationary
  • Experienced in primary care: internal medicine, geriatrics, or family medicine, with a minimum of one year of relevant work experience
  • Current BLS/ACLS certification
  • Driven to and comfortable with managing a primarily Medicare-aged, medically underserved patient population with comorbid conditions
  • Strong understanding of examination methodologies and diagnostics
  • Able to demonstrate interpersonal skills to be approachable and understandable to patients, families, and center team members
  • Confident in their ability to deliver high quality primary care, which includes being available to patients and addressing their medical and social needs
  • Passionate about their work and energized by being part of a fast growing organization
  • Comfortable traveling within the market to multiple centers within short distance of each other
