Job description

Nurse Practitioner is academically and clinically prepared to provide primary health care services within the scope of advanced nursing practice. The NP provides primary health care in collaboration with members of the health care team, according to established CCHC protocols.
Responsibilities:
  • Works as a part of a core health care team to provide health care in a coordinated multidisciplinary construct.
  • Complies with Agency, local, national and federal policies, procedures, directives and mandates for provision of medical care at CCHC.
  • Complies with all directives, actions and responsibilities as outlined in Agency and Patient Centered Medical Home policies and procedures.
  • Precepts health sciences students and participates in faculty and university affiliated activities as indicated.
  • Initiates and/or participates in ongoing research at CCHC, where applicable.
  • Participates/leads appropriate interdisciplinary care teams.
  • Participates in the development and review of CCHC policy and procedure development as appropriate.
  • Participates in and complies with all peer review and performance appraisal processes, policies and procedures.
  • Meets scheduled health center times and productivity standards.
  • Meets all requirements necessary to maintain: a valid license including re-certification, and required CEUs.
  • Participates in CCHC staff meetings, Quality Assurance and other committees, and community education programs.
  • Participates in orientation of new staff.
  • Provision of administrative assistance to supervisors.
  • Participates as an alternate/back up medical provider for special projects when needed and when appropriate and available (i.e. Back to School Health Fair, Mobile Van Outreach, etc.)
  • Other duties as assigned
Minimum Qualifications:
REQUIRED
  • Licensed in Illinois as an advanced practice nurse and controlled substance provider
  • Master’s Degree in Nursing (MSN) for Nurse Practitioners.
  • Valid DEA certificate
  • Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification
  • Excellent clinical skills, excellent communication and interpersonal skills, and demonstrates team-oriented approach to health care delivery.
DESIRED
  • Bilingual (English/Spanish)
  • Board Certification as a Nurse Practitioner by a nationally accredited organization within one (1) year of commencing employment as a Nurse Practitioner at CCHC or from the date of graduation from the MSN program whichever is the later
  • Experience treating chronic illnesses (HIV, Sickle Cell)
  • Outpatient primary care experience
