Job description

Company Description Bristol Hospice provides comprehensive hospice services to patients and families facing serious illness, with a focus on compassionate, respectful, and high-quality care. An interdisciplinary care team—including nurses, physicians, home health aides, social workers, counselors, spiritual care providers, therapists, dietitians, volunteers, and bereavement specialists—develops individualized plans of care to support physical, psychosocial, and spiritual needs. Care is delivered wherever patients call home, including private residences, hospitals, nursing facilities, and hospice centers. Services range from nursing care, physician services, symptom management, counseling, and spiritual support to therapy, dietary services, medical equipment, supplies, volunteer support, and bereavement care. With locations across multiple states, Bristol Hospice partners with families, caregivers, healthcare professionals, and referral sources to provide trusted, compassionate hospice care in the communities it serves.

Role Description The Hospice Liaison is a full-time, on-site role based in Portland, OR, responsible for building and maintaining relationships with healthcare providers, facilities, and community partners to support timely and appropriate hospice referrals. This role includes educating referral sources and families about hospice services, eligibility, and benefits, and serving as a resource for end-of-life care options. The Hospice Liaison conducts outreach visits, participates in community events, and collaborates closely with the clinical team to coordinate admission processes and ensure a smooth transition to hospice care. Day-to-day tasks include assessing referral needs, communicating patient information to the interdisciplinary team, providing responsive follow-up, and documenting activities in accordance with organizational and regulatory standards. The position requires consistent on-site presence within the Portland area to support patients, families, and referral partners.

Qualifications

  • Demonstrated knowledge and experience in Hospice Care, End-of-Life Care, and Palliative Care.
  • Strong Sales skills, including relationship-building, outreach, and referral development.
  • Understanding of Home Care services and coordination of care across settings.
  • Knowledge of hospice regulations, eligibility criteria, and healthcare industry practices.
  • Excellent interpersonal, communication, and presentation skills with diverse stakeholders.
  • Ability to work collaboratively with interdisciplinary clinical teams and community partners.
  • Experience in healthcare liaison, outreach, or similar patient-facing role preferred.
  • Relevant clinical or healthcare background (e.g., nursing, social work, allied health) strongly beneficial.
  • Proficiency in documentation, basic computer systems, and maintaining accurate records.
  • Valid driver’s license and ability to travel locally between healthcare facilities and community sites.
