ALIYA of Evanston is seeking a dedicated and clinically skilled Infection Preventionist / Wound Care Nurse to join our team. This position plays a critical role in overseeing the facility's Infection Prevention and Control Program while providing comprehensive wound care management to promote positive resident outcomes and regulatory compliance.

Duties & Responsibilities
Infection Prevention
Oversee and maintain the facility's Infection Prevention and Control Program
Conduct infection surveillance, tracking, trending, and reporting
Monitor and investigate infections, outbreaks, and communicable diseases
Ensure compliance with CMS, IDPH, CDC, and OSHA infection prevention guidelines
Develop and update infection prevention policies and procedures
Educate staff on infection prevention practices, hand hygiene, PPE usage, and isolation precautions
Participate in QAPI, Quality Assurance, and Performance Improvement initiatives
Prepare reports and documentation for surveys and regulatory inspections
Wound Care
Assess, monitor, and manage resident wounds, pressure injuries, skin conditions, and treatment plans
Collaborate with physicians, nurse practitioners, and interdisciplinary team members to develop and implement wound care interventions
Perform wound measurements, documentation, and ongoing evaluations
Monitor wound healing progress and recommend treatment modifications as needed
Educate nursing staff on wound prevention and treatment protocols
Participate in skin rounds and clinical meetings
Ensure accurate and timely documentation of wound care services
Qualifications
Current Registered Nurse (RN) license in the State of Illinois required
Wound Care Certification (WCC, WOCN, or equivalent) preferred
Infection Prevention certification (CIC) preferred or willingness to obtain
Minimum of 2 years of skilled nursing, long-term care, sub-acute, or wound care experience preferred
Knowledge of CMS, IDPH, CDC, and infection prevention regulations
Strong assessment, clinical, communication, and organizational skills
Experience with electronic medical records and clinical documentation systems
