• Conduct thorough evaluations of patients' physical capabilities, identifying their unique goals and needs to craft individualized treatment plans.
• Deliver specialized therapy interventions to our geriatric patients, utilizing therapeutic exercises, manual therapy techniques, and modalities tailored to enhance recovery and independence.
• Monitor patient progress with regular assessments, documenting findings and modifying treatment protocols to achieve optimal therapeutic outcomes.
• Collaborate with a multidisciplinary team, including physicians, occupational therapists, and speech therapists, to ensure a cohesive and comprehensive approach to each patient's care plan.
• Provide education and resources for patients and their families regarding therapeutic modalities, home exercise programs, and strategies for injury prevention.
• Design and implement fall prevention strategies and safety measures, contributing to overall patient safety within our assisted living community.
• Stay informed on advancements in physical therapy practices, participating in continuing education opportunities to enhance your clinical skills.
• Uphold ethical and professional standards, ensuring confidentiality and a commitment to delivering the highest level of care.

Your role at Agile Stride Rehab will primarily focus on a geriatric population, where you will have the opportunity to impact the lives of individuals who have made significant contributions to society. Our patients often deal with age-related challenges such as mobility limitations, chronic pain, and post-operative rehabilitation needs. By focusing on this specific demographic, you will play an integral part in helping them navigate their physical rehabilitation journeys.

• A graduate of an accredited physical therapy program, with a Bachelor’s degree or higher in Physical Therapy.
• Active licensure to practice as a Physical Therapist in Texas.
• A strong commitment to patient-centered care, focusing on enhancing independence and quality of life for our patients.
• Exemplary interpersonal and communication skills to effectively liaise with patients, families, and the wider healthcare team.
• Familiarity with electronic medical records (EMRs) and documentation processes relevant to patient care.
• Knowledge and application of evidence-based practices in the field of physical therapy.
• The ability to work autonomously and as part of a multidisciplinary team, ensuring comprehensive and coordinated patient care.

• Experience in geriatric therapy is highly desirable, enhancing your ability to relate to and effectively care for our patient population.
• Familiarity with advanced therapeutic techniques and emerging trends in physical rehabilitation.
