About the Role:

As a Nurse Practitioner in post-acute care, you will provide in-person care to patients in skilled nursing and rehabilitation facilities. In this role, you will partner with facility teams and Trumen-affiliated physicians to manage chronic and acute conditions, support transitional care planning, and contribute to quality-improvement initiatives within your designed facilities.


Core Responsibilities:

  • Collaborate with Trumen physicians and facility medical directors to coordinate patient care
  • Conduct comprehensive clinical assessments and manage treatment plans
  • Support transitional care planning to help prevent avoidable hospital readmissions
  • Reconcile medications and provide patient education consistent with scope of practice
  • Foster positive working relationships with facility leadership, clinical staff, and care coordinators
  • Collaborate as applicable Trumen-affiliated value-based and quality improvement initiatives
  • Serve as a liaison between Trumen and disignated skilled nursing facilities (SNFs)
  • Maintain accurate and timely documentation within the EMR
  • Maintain an on-site presence consistent with your agreed schedule and facility needs

Qualifications:

  • Active Nurse Practitioner license in the state of Texas
  • Minimum of 1 year experience as a Nurse Practitioner, preferably in a post-acute, SNF, or geriatric care setting
  • Ability to work independently while effectively collaborating with the care team
  • Strong interpersonal skills and a passion for caring for complex patients
  • Reliable transportation within the Kerrville area
