Job description

Work Dates Needed: Starting anytime asap - 3 months or until a permanent provider is found
Preferred Schedule: M-F 8a - 5p, 1 in 4 call rotation which is all phone based
Worksite Setting: PACE outpatient center based with some in-home visits (SNF & AFC homes)
Scope of Work: Diagnosis and treat a nursing home eligible senior population who desires to remain living independently, function as part of the interdisciplinary team to care plan for agreed upon participant goal outcomes. Roughly 5 to 10 patients a day seen. Provider sees patients in Short term living & Adult Foster Care homes.
Provider is a part of the 11-discipline interdisciplinary team (includes OT,PT, Speech Therapy, Pharmacy, Social Work, Home Care (skilled and unskilled), Day Center, Primary Care Clinic, Transportation, Nutrition) together see around 60 patients a day,
Licenses, Certifications, Requirements: Michigan licensure, ACLS/BLS, geriatric population experience required, PACE experience required, must be board Certified & DEA. Will accept FP IF they have PACE / Geriatric experience.

Rates: $145 hour plus we bill travel / lodging expenses
