Advanced Practice Provider (PA or NP)
Position Type: Walk-In Primary Care Services
Schedule: Friday-Sunday | 9:00 a.m. - 7:00 p.m.
Location: Muskegon, MI
About Hackley Community Care:
Located along the beautiful shores of Lake Michigan, HCC is a Federally Qualified Health Center providing integrated medical, dental, behavioral health, and pharmacy services to residents in Muskegon County and surrounding areas. We are seeking a mission-driven Advanced Practice Provider committed to promoting health, preventing disease and improving lives.
Position Summary:
The Advanced Practice Provider delivers high-quality medical care in accordance with accepted professional standards. Responsibilities include patient evaluation, treatment, referrals, and thorough documentation. This role supports both scheduled and walk-in primary care services.

Key Responsibilities:
  1. Serve as an active member of the care management team, providing leadership and clinical guidance
  2. Deliver comprehensive primary care within scope of PA/NP training

Qualifications:
