Job Title: Program Nurse - LPN Department: Adult Day Program Supervisor: Program Director Part Time: 16 to 22.5 hours per week (2 to 3 days per week) Job Objective: Coordinates delivery of nursing and social services for adult day participants Minimum Qualifications Education: Graduate of accredited nursing school Experience: 2-3 years of nursing experience. Experience working with the elderly and those with dementia/Alzheimer's Disease, preferred work in a community-based setting and knowledge of services and entitlements for frail elderly are a plus. Licensure: State of Connecticut Nursing License; CPR certification Job Standards 1. Demonstrates clear, compassionate, and effective nursing, demonstrating excellent judgement. 2. Communicates effectively with clients, employees, and care partners. 3. Ability to maintain positive working relationships both within the organization and community. 4. Empathetic, culturally aware, inclusive, and person-centered of all we serve. 5. Respect client privacy and dignity. 6. Creative, supportive, and educational approach to working with clients with a variety of needs and reactions to new life circumstances. 7. Ability to juggle multiple tasks and accomplish deadlines in an organized fashion. 8. Utilizes Microsoft suite, including outlook, and excel; flexibility and initiative to adapt to changing software technology. 9. Ability to travel to client homes and other community sites as necessary related to networking and intakes. 10. Availability of personal vehicle for work related travel (mileage reimbursed). Travels primarily within Fairfield and New Haven counties. 11. All other related duties as assigned. Essential Job Functions 1. Conduct new client intakes and assessments to determine the appropriateness of referrals and encourage participation at start of program and once enrolled. 2. Conduct/assess the nutritional, medical, physical and mental functional needs of the clients during the initial assessment and ongoing as necessary within scope of nursing practice.
