Schedules Available:
8:30 AM to 4:00 PM, Monday - Friday (full-time/35 hours)
Momentum is hiring Caregivers/Direct Support Professionals (DSP) for our new Adult Day Program in Thousand Oaks.
As a Caregiver/Direct Support Professional, you will support disabled individuals with daily living needs in an Adult Day Program setting. As a Caregiver/DSP, you will support people with their physical, emotional, and social needs by focusing on the person first. Our Caregiver/DSP's have the unique opportunity to build a rewarding career in healthcare and to develop caring relationships.

Caregiver/DSP Essential Responsibilities:
  • Promotes a friendly, comfortable, and safe environment
  • Respects and promotes the right of individuals
  • Assist people attending the program with program activities and community-based activities
  • Assist people attending the program with communication, feeding, mobility, toileting, hygiene, and other activities of daily living
  • Assists with moving in and out of wheelchairs, automobiles, pool, etc.
  • Follow each person's Individual Service Plan (ISP) goals
  • Document behavioral and daily living skills data of people supported accurately and in a timely manner
  • Document individuals' activity participation and critical issues in a timely manner
  • Follows and implements company policies
  • Attends required in-service trainings
  • Performs other duties as assigned to meet department and/or business need

Caregiver/DSP Qualifications:
  • Must be at least 18 years of age
  • Valid and current proof of legal right to drive in California; required
  • Current First Aid and/or CPR Certificate; preferred, not required
  • High School Diploma or equivalent; required
  • Experience working with developmentally disabled individuals; preferred
  • Participate in pool therapy and aquatic activities
  • Read, write, comprehend, and communicate in English well enough to perform essential duties and responsibilities
  • Ability to maintain confidentiality
  • Communicate and work professionally and cooperatively with others
  • Successfully pass all post offer, pre-employment screening requirements, including criminal background check, drug screening, health screening, TB screening and provide proof of eligibility to work in the United States
  • Ability to run, push, pull, bend, squat, kneel, and lift up to fifty (50) pounds
