An admission into healthcare is more than paperwork - it is a meaningful transition for a resident and their family. At Greencroft Goshen, we are looking for an experienced Healthcare Admissions Coordinator who can make that transition feel organized, reassuring, and welcoming from the very first conversation.

In this highly collaborative role, you will connect prospective residents and families with the care they need while partnering with healthcare leaders, clinical teams, referral sources, and community professionals. Your work will directly support our commitment to One Team, One Mission as we create a coordinated and compassionate admission experience.

Schedule

  • Monday through Friday
  • 8:00 a.m.4:30 p.m.
  • Some weekend or later hours may be needed based on admissions activity.

What Youll Do

Guide residents and families

  • Respond promptly and professionally to inquiries about Greencrofts healthcare services.
  • Conduct prospect interviews and tours with prospective residents and their families.
  • Communicate admission requirements, placement information, and next steps clearly.
  • Follow up with new residents and their families during their first week to help ensure a positive transition.
  • Provide basic information about Medicare and Medicaid and assist families with the online Medicaid application process when needed.

Coordinate the admissions process

  • Oversee healthcare admissions from the initial inquiry through placement.
  • Collect required information and ensure paperwork, assessments, authorizations, and notifications are completed.
  • Collaborate with medical, nursing, and marketing teams to determine the most appropriate resident placement.
  • Maintain accurate information about bed availability and available community services.
  • Work with insurance companies to verify benefits and obtain pre-certification.
  • Ensure pre-admission screening requirements are completed, including PASRR Level II and LDC requirements.
  • Assist staff with online PASRR assessments and maintain an accurate PASRR census.

Build community relationships

  • Develop and maintain strong relationships with hospital discharge planners, case managers, medical professionals, insurance representatives, senior organizations, and other referral partners.
  • Make outreach calls to share information about bed availability, services, and new programs.
  • Maintain regular communication with Greencroft Resident Service Coordinators to help identify and meet the needs of campus residents.
  • Partner with the Marketing department to promote healthcare services and respond to referrals.

Support occupancy and process improvement

  • Help manage occupancy levels, census, and payer mix.
  • Maintain an up-to-date database of current and potential referral sources.
  • Manage waiting lists, applications, follow-up calls, and immediate-needs lists.
  • Facilitate Admissions Team communication and follow through on recommendations.
  • Track admissions data and report referral-to-admission conversion ratios.
  • Identify technology and process improvements that can make admissions more efficient.

What Youll Bring

  • Previous experience with admissions in a long-term care setting is required.
  • Knowledge of long-term care practices and Medicare and Medicaid procedures is required.
  • Bachelors degree preferred.
  • Strong communication, relationship-building, organization, and follow-through skills.
  • The ability to handle sensitive conversations with professionalism and compassion.
  • A team-centered approach and the ability to coordinate effectively across multiple departments.
