Admissions Coordinator – Skilled Nursing

Allegria Village | Dearborn, MI

Position Summary

Allegria Village is seeking an experienced and compassionate Admissions Coordinator to join our Skilled Nursing team. The Admissions Coordinator plays a key role in helping prospective residents and their families navigate the Skilled Nursing admission process, from initial inquiry and clinical screening through admission and follow-up.

This position works closely with patients, families, hospitals, physicians, healthcare professionals, and internal departments to ensure a smooth, timely, and positive admission experience. The ideal candidate will have at least one year of Skilled Nursing Facility (SNF) admissions experience, excellent communication and organizational skills, and the ability to manage multiple priorities in a fast-paced healthcare environment.

Key Responsibilities

  • Coordinate and manage the Skilled Nursing admissions process from initial inquiry through admission.

  • Obtain and review applicant information, including applications, medical records, clinical information, and financial documentation.

  • Screen prospective residents to determine whether their clinical needs meet facility admission criteria.

  • Communicate admission criteria, services, and expectations clearly to patients, families, referral sources, and healthcare professionals.

  • Coordinate with hospitals, physicians, discharge planners, social workers, families, and other referral sources to obtain the information necessary for admission.

  • Complete and maintain accurate admission and financial responsibility documentation.

  • Coordinate admission logistics and support services, including transportation and other resident needs.

  • Conduct tours of the Skilled Nursing facility and provide information about Allegria Village, available services, and the admission process.

  • Promote the Skilled Nursing services and programs at Allegria Village through professional relationships, informational materials, tours, and other outreach efforts.

  • Build and maintain positive relationships with hospitals, healthcare professionals, community organizations, residents, and families.

  • Respond to questions and concerns from prospective residents and their families and work to resolve concerns in a professional and timely manner.

  • Maintain accurate admissions records and prepare reports related to admissions activity, trends, and outcomes.

  • Maintain strict confidentiality of resident, medical, financial, and organizational information.

  • Follow all applicable safety policies, procedures, and healthcare regulations.

  • Stay current with Skilled Nursing admissions practices, regulations, and industry standards through continuing education and professional development.

  • Work collaboratively with the Director of Marketing and other departments to support census development and overall organizational goals.

  • Take initiative in identifying opportunities to improve the admissions experience and contribute to the success of the Skilled Nursing community.

Qualifications & Skills

  • Minimum of 1 year of Skilled Nursing Facility (SNF) admissions experience required.

  • Strong knowledge of the Skilled Nursing admissions process and requirements.

Education
