Position Summary

We are seeking a detail-oriented Medical Assistant to support our participation in an Accountable Care Organization (ACO) program.This role blends traditional clinical support with population health, quality reporting, and coding-related responsibilities. The ideal candidate understands CPT and ICD-10 diagnosis codes and is comfortable working with care coordination and quality measures.

Key Responsibilities

  • Support ACO initiatives including quality measures, care gaps and population health workflows
  • Assist with accurate documentation and review CPT and ICD-1- diagnosis codes
  • Identify and flag care gaps (e.g., preventative screenings, chronic condition follow-ups)
  • Coordinate patient outreach related to ACO quality measures and annual wellness visits
  • Assist providers with clinical documentation to support accurate coding and compliance
  • Maintain accurate patient records in the EHR
  • Communicate with patient regarding appointments, labs, referrals, and care plans
  • Collaborate with clinical and administrative staff to meet ACO performance goals
  • Ensure compliance with HIPAA and regulatory requirements

Qualifications

  • Certified or registered Medical Assistant preferred
  • Experience in working a primary care or value-based care setting
  • Working knowledge of CPT and ICD-10 diagnosis codes
  • Familiarity with ACO, Medicare, or value-based care programs
