Work Environment

This position is primarily remote, allowing you to work from the comfort of your home while maintaining a collaborative relationship with other healthcare professionals. Key Responsibilities
  • Conduct thorough assessments and evaluations of patientsโ€™ mental health status utilizing instruments such as the Diagnostic and Statistical Manual of Mental Disorders (DSM) to create individualized care plans that respect cultural diversity and address special needs.
  • Monitor and assess patient progress continuously, modifying treatment plans as necessary to reflect changes in patients' conditions and to ensure effective interventions.
  • Prepare and maintain accurate clinical documentation, ensuring compliance with all regulatory requirements and department standards, utilizing advanced electronic health record systems.
  • Collaborate closely with patients and their families to formulate comprehensive treatment plans, emphasizing safety and symptom reduction, while integrating their unique goals and aspirations.
  • Provide coordination of referral options to connect patients with necessary treatment resources, fostering strong relationships with other clinicians and referral agencies for seamless care integration.
  • Engage in mentoring and educational practices for interns, students, and residents, contributing to the growth of future healthcare providers.
  • Under the guidance of your clinical supervisor, participate in peer review, chart auditing, and clinical collaboration, aimed at enhancing team performance and professional development.
Patient Population / Client Focus

This role specifically targets individuals and families navigating the challenges associated with cancer diagnoses. As a key member of our hospice team, you will address both the psychological and emotional needs of patients, actively working to enhance their quality of life through tailored therapeutic interventions that recognize their unique circumstances and backgrounds.

Qualifications
  • A Masterโ€™s Degree in Social Work from an accredited institution.
  • Current State License as a Licensed Clinical Social Worker (LCSW) confirmed and in good standing.
Preferred Experience
  • Experience in oncology or chronic illness care is preferred, though not mandatory.
  • Demonstrated proficiency in using electronic health records and telehealth platforms.
  • Familiarity with evidence-based therapeutic methods suited for working with diverse patient populations.
Compensation and Benefits

Health Access Medical Center offers a competitive salary range of USD 90,000 to 105,000 per year, commensurate with education and experience.
