Located in the heart of Zanesville, The Oaks Rehabilitation and Healthcare Center is a leading skilled nursing facility that provides compassionate, high-quality care to our residents. We are currently seeking a Registered Nurse (RN) to join our caring team and contribute to the well-being of our residents.

Position Overview:
As a Registered Nurse (RN) at The Oaks, you will be at the forefront of patient care, working directly with residents to assess their needs, administer treatments, and ensure their overall health and comfort. You will collaborate with a multidisciplinary team of healthcare professionals to develop personalized care plans, monitor patient progress, and provide education and support to patients and their families.

Key Responsibilities:

     Conduct comprehensive assessments of patients' physical and mental health.

     Administer medications and treatments according to prescribed guidelines.

     Develop and update individualized care plans tailored to residents' needs.

     Monitor and document patient progress, including changes in condition or behavior.

     Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated care.

     Provide education and emotional support to residents and their families.

     Supervise and support nursing staff, including LPNs and CNAs, in the delivery of patient care.

     Ensure adherence to facility policies, state regulations, and federal healthcare standards.

     Assist with patient admissions, discharges, and transfers.

     Maintain accurate, up-to-date patient records in compliance with HIPAA standards.

Qualifications:

     Current Registered Nurse (RN) license in the state of Ohio.

     Previous experience in a skilled nursing facility or long-term care setting is preferred, but not required.

     Strong knowledge of nursing practices, clinical procedures, and regulations.

     Excellent interpersonal and communication skills.

     Compassionate, patient-centered approach to care.

     Ability to work in a fast-paced, team-oriented environment.

     Basic computer skills and familiarity with electronic health records (EHR).
