Job description

Overview
• Department: Pharmacotherapy Clinic
• Schedule: Day shift- HYBRID (Split time between remote/onsite)
• Facility: Olympia Fields
• Location: Olympia Fields, IL 60461
**Hybrid role (Split time between onsite and remote) Looking to focus on applicants that live in the Chicago/Olympia Fields and surrounding areas
**Ambulatory Care and experience with chronic disease management will be crucial to success (i.e. diabetes, hypertension, dyslipidemia).
**Experience in 340B
Responsibilities
The Ambulatory Clinical Pharmacist is responsible for providing patient care and advanced clinical pharmacy services in the ambulatory pharmacy space, including referral based HOPD pharmacist managed clinics, specialty pharmacy, and population health pharmacy services. This role involves collaborating with prescribers in the ambulatory setting and providing direct patient care managing medication therapy. The clinical pharmacy specialist ensures effective communication with prescribers regarding identified medication related problems and potential solutions and documents patient interactions across various electronic medical record (EMR) platforms.
Qualifications
EDUCATION, EXPERIENCE, TRAINING
Required qualifications:
1. Current state Registered Pharmacist License.
2. B.S. in Pharmacy or PharmD degree required.
3. PGY-1 Pharmacy Practice Residency required (significant experience may be considered in lieu of residency)
4. Board certification (BCPS, BCACP, or equivalent) within 18 months of hire required.
5. Strong verbal and written communication skills required.
6. Strong customer service skills required.
Preferred qualification:
1. PGY-2 residency in ambulatory care or a related specialty preferred.
2. Expertise in 340B program operations and compliance preferred.
3. Proficiency in EPIC or similar EMR systems preferred.
4. Advanced clinical knowledge in specialty areas such as diabetes, cardiology, pulmonology, rheumatology, or oncology preferred.
5. Experience as a pharmacy residency preceptor in ambulatory pharmacy services preferred.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $115,107.20 to $166,712.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
