Per Diem Home Health OASIS Registered Nurse (R.N.)

Job Summary:
We are seeking a compassionate and skilled Per Diem Home Health OASIS Registered Nurse (R.N.) to join our team of dedicated healthcare professionals. As a Per Diem OASIS R.N., you will be an integral part of our home health team, providing high-quality, patient-centered care to our clients in their homes. This is a per diem position, with flexible scheduling to meet the needs of our clients.
Responsibilities:
Conduct comprehensive assessments of patients in their homes, identifying their needs and developing individualized care plans to meet those needs
Provide skilled nursing care to patients, including wound care, medication management, and education
Collaborate with other healthcare professionals, including therapists, social workers, and home health aides to ensure comprehensive care
Maintain accurate and detailed documentation of patient care, including OASIS assessments and progress reports
Communicate regularly with patients, families, and healthcare teams to ensure continuity of care and address any concerns
Participate in ongoing education and training to stay current with industry developments and best practices
