Regional Hospital Liaison โ€“ Central Ohio Region

Garden Springs Healthcare is seeking a highly driven and relationship-focused Regional Hospital Liaison based in the Columbus, Ohio market. This fully external position is responsible for developing and maintaining strong referral relationships with hospital systems, case managers, discharge planners, physicians, and healthcare partners throughout the Central Ohio region.

We are looking for a proven performer with existing hospital relationships and a strong understanding of post-acute care operations, admissions, and patient transition management within the SNF and/or LTACH space.

Position Summary

The Regional Hospital Liaison will serve as the primary field representative for Garden Springs Healthcare throughout the Columbus market. This role requires daily in-person hospital visits, strategic relationship development, and consistent communication with referral sources to drive census growth and strengthen market presence.

The ideal candidate is well-connected within Central Ohio hospitals, highly organized, self-motivated, and comfortable operating independently in a fast-paced healthcare environment.

Key Responsibilities
  • Develop and maintain referral relationships with hospitals, physicians, case managers, social workers, and discharge planners throughout Central Ohio

  • Conduct daily in-person visits to assigned hospital accounts

  • Generate and manage referral pipeline opportunities for skilled nursing and post-acute placements

  • Coordinate closely with admissions, clinical, and operational teams to ensure smooth patient transitions

  • Educate referral partners on Garden Springs Healthcare services, capabilities, and outcomes

  • Identify new business opportunities and strategic partnerships within the market

  • Attend networking events, hospital meetings, and community outreach initiatives

  • Maintain detailed documentation of referral activity, account management efforts, and relationship development

  • Meet and exceed established census and referral growth goals

Qualifications
  • Minimum 5+ years of experience in Skilled Nursing Facility (SNF), Long-Term Acute Care Hospital (LTACH), or post-acute healthcare marketing/business development

  • Existing relationships and proven experience working with Columbus-area hospital accounts required

  • Demonstrated track record of successful referral development and census growth

  • Strong understanding of hospital discharge planning and post-acute care continuum

  • Excellent communication, relationship-building, and presentation skills

  • Highly self-motivated with the ability to work independently in a fully external role

  • Experience with CRM systems and referral tracking preferred

  • Valid driverโ€™s license and reliable transportation required
