Position Overview

Harmonic Motion PT is actively seeking a dedicated Full-Time Geriatric Physical Therapist to join our team in Woodbridge, New Jersey. The ideal candidate will possess a strong commitment to improving the quality of life for our geriatric patients through personalized therapy plans and compassionate care. This role will require a thorough understanding of geriatric therapy, effective patient communication skills, and the ability to collaborate within a multidisciplinary healthcare environment.

Work Environment

This is an onsite position, focusing primarily on home care within the local Woodbridge community. You will engage directly with patients in their home environments, allowing for tailored therapy sessions that consider their personal surroundings. Our work setting fosters a supportive atmosphere where our therapists can thrive, backed by a solid administrative team that assists with scheduling and documentation. The standard work schedule runs from Monday to Friday, 9 AM to 5 PM, providing a balanced work-life framework for our employees.

Key Responsibilities
  • Conduct thorough evaluations of patients' physical capabilities, developing individualized treatment plans that cater to their specific needs and rehabilitation goals.
  • Deliver skilled therapeutic interventions such as exercise programs, manual therapy techniques, and various modalities tailored to geriatric patients.
  • Regularly monitor and document patient progress, making necessary adjustments to treatment modalities to ensure optimal recovery outcomes.
  • Collaborate closely with a multidisciplinary team of healthcare professionals, including physicians, occupational therapists, and speech therapists, to create a holistic care approach for each patient.
  • Educate patients and their families on therapeutic techniques, such as home exercise programs and injury prevention strategies, ensuring they know how to maintain their health effectively.
  • Implement and oversee fall prevention programs and safety measures in patients’ homes, critical for reducing fall risks within the geriatric population.
  • Stay current with advancements in physical therapy through continuous education and professional development opportunities, enhancing both clinical skill sets and service quality.
  • Maintain a high standard of ethical practices, continuously protecting patient confidentiality and delivering empathetic care.
Patient Population / Client Focus

Qualifications
