SUMMARY OF POSITION:

EPIC is an organization thatstrives to develop and maintain a harmonious work environment and encouragesgrowth and professional development. All EPIC team members have basic requirementsin areas such as Leadership, Team Management, andCompliance Assurance.

The position oftheHomeCareCoordinatorisresponsible forcoordinating client care services, caregiver scheduling, compliance monitoring, referral relationship management, and operational support to ensure exceptional service delivery. This role serves as a vital link between clients, caregivers, referral sources, and leadership, helping to drive organizational growth whilemaintainingregulatory compliance and quality standards. The Home Health Care Coordinator works collaboratively with the Director of Nursing, Recruiter, CEO, caregivers, clients, and referral partners to support EPIC's mission of delivering outstanding home healthcare services.Ensures that EPIC's existing database structure isutilizedbyCAREGIVERsas required by regulation.Overall, ensures EPIC's in-home care servicesoperatesmoothly.

QUALIFICATIONS &REQUIREMENTS:

  • Associate degree in Healthcare Administration, Business Administration, Nursing, Human Services, or a related field preferred.
  • Equivalentcombination of education and experience may be considered.
  • Minimum of 2 years of experience in home health care, healthcare coordination, scheduling, staffing, or healthcare operations preferred.
  • Experience working with caregiver management systems and electronic health record platforms preferred.
  • Knowledge of home care regulations and compliance requirements preferred.
  • Musthave a dependable automobile and a clean driving record.
