Pay Rate: $37.00/hr

Key Responsibilities:

  • Provide care for adult and pediatric patients in New York City through home visits, monitoring, and treatment.

  • Conduct assessments and treatment under APP supervision for patients with limited access to healthcare.

  • Maintain accurate patient records using HealthPoint and Athena EMR systems.

  • Practice with telemedicine support and ensure compliance with clinical standards.

  • Act as a conduit for medical treatment in Health Plan Partnerships.

  • Engage in health management, medication, and care goal discussions.

  • Provide supervised treatment for common diagnoses.

  • Participate in ongoing education and adhere to safety protocols.

  • Deliver compassionate, patient-centered care.

Qualifications:

  • Minimum of two years of experience as a Licensed Practical Nurse (LPN)

  • Active and current American Heart Association BLS certification required

  • Experience in Emergency Department, Urgent Care, Internal Medicine, Critical Care, Family Practice, or other relevant areas preferred

  • Active and current driver's license for 2+ years required

  • Current unrestricted NY State LPN license required

  • Clinical work experience with pediatrics required

  • Phlebotomy experience required

  • Fluent in Spanish preferred

  • Ability to lift 50-60 lbs
