  • Call Schedule:1:6 weeks rotation
  • Practice Detail: Established Outpatient clinic working with physicians and APP treating a Geriatric population in the community.
  • Facility: Ascension Living PACE Michigan
  • Location: Flint, Michigan
  • This job location is currently located in a Health Professional Shortage Areas (HPSAs) and/or Medically Underserved Areas/Populations (MUA/P) Shortage Designation Type.

What You Will Do
Ascension Living PACE Michigan is looking to add a Full Time Geriatrics Physician at our Flint Michigan Clinic.
Position Overview:
  • Evaluate and treat patients with appropriate medical diagnostic and treatment skills. Coordinate discharge planning with case management.
  • Recommend, participate in, and work to ensure the success of efforts to improve cost effectiveness and quality of care provided to patients.
  • Accept patient referrals from other physicians. Acts as admitting, attending and/or consulting physician for patients who require hospitalization for primary medical diagnoses, depending on needs of patients, medical staff and hospital.
  • Participate in academic programs (eg medical education and research) as requested.
  • Provide appropriate documentation that meets insurance company requirements; appeal all denials in a timely manner.
  • Participate in inpatient and outpatient consultation for specialty, which may include on call schedules.

Job Duties:
  • Will provide services at Ascension Living Flint, MI clinic, and could include rounding on inpatients from time to time at various Ascension Living Flint long term care facilities.
  • Typically see 4-6 patients in a clinic setting (2-3 routine assessments, plus 2 urgent visits daily)
  • Family Medicine trained with Geriatric experience is a plus, but open to newly trained family medicine residents
  • Experience in simple office procedures such as skin biopsy, joint injections, etc. Additionally experience in having goals of care/advanced care planning conversations and palliative care/hospice experience would be a bonus (but not required).
  • We have a primarily medically complex patient population, so experience with managing such patients would be great. Also, a willingness to evaluate patients in their home setting or facility as these situations occur on a fairly frequent basis.
  • Willingness to work in a team setting (with an interdisciplinary team) and flexibility with the needs of our patients, as situations will change quickly, and we may need to change plans for them based on what is going on

About PACE:
  • The Program for All Inclusive Care (PACE) is an alternative to nursing home care and - through an interdisciplinary care team of physicians, nurses, social workers, therapists, and aides - coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.
  • Aged 55 years old or older.
  • Live in a PACE provider's service area.
  • Certified by the state as meeting the need for the nursing home level of care.
  • Able to live safely in the community with the help of PACE services at the time you enroll.
  • While there are no financial eligibility requirements, PACE programs are covered by Medicare and Medicaid for eligible individuals.
  • PACE services are also available on a private pay basis.

What You Will Need
Licensure / Certification / Registration:
  • Physician MD/DO obtained prior to hire date or job transfer date required.
  • Advanced Life Support preferred. American Heart Association or American Red Cross accepted.

Education:
  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
