Job description

AMM is seeking a Field Care Manager (FCM) to support our growing Independent Practice Association (IPA) network across New York. This is a field-based, hybrid role designed for clinically experienced, bilingual professionals who work comfortably at the intersection of patient care, provider engagement, and operational execution.

The Field Care Manager will serve as a key liaison between medical practices staff and the IPA, supporting care gap closure, quality improvement, care management workflows, and the sharing of best practices—particularly within Korean-speaking patient populations.

This role requires regular in-person visits to practices, with additional remote coordination and administrative work.

Key Responsibilities

  • Act as the primary liaison between AMM/IPA leadership and assigned medical practices
  • Support providers and staff in closing care gaps (AWVs, screenings, chronic disease management)
  • Assist with care management coordination and patient outreach efforts
  • Share best practices related to quality metrics, documentation, and clinical workflows
  • Provide in-office support to clinical and front-desk teams as needed
  • Facilitate communication between practices and IPA operations, quality, and data teams
  • Support implementation of value-based care initiatives and performance improvement programs
  • Track progress, surface challenges, and provide feedback from the field
  • Participate in hybrid work activities including reporting, planning, and virtual meetings

Qualifications

  • Clinical or medical office background required (RN, MA, PA, or former medical office staff strongly preferred)
  • Fluent in English and Korean (required)
  • Experience in primary care, care coordination, population health, or value-based care preferred
  • Comfortable engaging directly with physicians and practice staff
  • Strong interpersonal, organizational, and communication skills
  • Willingness to travel regularly between practices across the New York metro area
  • Self-directed, reliable, and comfortable working independently in a field-based role
  • Familiarity with EMRs and standard productivity tools (Microsoft/Google)
