As an RN Case Manager, you'll coordinate, manage, and deliver individualized care to patients in their homes while serving as a key member of an interdisciplinary healthcare team.
  • Develop, implement, and manage individualized patient care plans.
  • Complete comprehensive nursing assessments and ongoing patient evaluations.
  • Provide skilled nursing care, treatments, and disease management education.
  • Monitor patient progress and adjust care plans to promote positive outcomes.
  • Coordinate services with physicians, therapists, social workers, and other healthcare professionals.
  • Educate patients and caregivers regarding diagnoses, medications, treatments, and self-management strategies.
  • Identify changes in patient condition and appropriately communicate recommendations to the care team.
  • Ensure compliance with Medicare, state, and organizational documentation standards.
  • Maintain accurate, timely electronic documentation.
  • Build strong, trusting relationships with patients and families while advocating for their healthcare needs.
  • Participate in care conferences, quality improvement activities, and interdisciplinary collaboration.

Requirements
  • Current Registered Nurse (RN) license in Iowa, in good standing.
  • Graduate of an accredited nursing program.
  • Valid driver's license, reliable transportation, and proof of automobile insurancethat meets VNA requirements.
  • Ability to travel throughout the service area.
  • Strong clinical assessment, communication, organization, and critical thinking skills.
  • Excellent communication, organization, and time management abilities.
  • Ability to work independently while collaborating effectively with physicians, patients, families, and interdisciplinary team members.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • All offers of employment are contingent upon the successful completion of a criminal background check conducted by the Iowa Division of Criminal Investigation (DCI) and verification of eligibility to work in healthcare programs, including a negative finding on the Office of Inspector General (OIG) Excluded Provider List.
Preferred
  • One year of clinical nursing experience preferred; home health, hospice, case management, community health, or public health nursing experience strongly preferred.
  • Case management experience.
  • Knowledge of Medicare home health regulations and documentation requirements.
  • Experience utilizing electronic medical records.
  • OASIS assessment experience.
