Work Environment

This position will be 100% remote, allowing for flexibility and work-life balance while still ensuring you can serve our clients effectively. The work will involve frequent, active communication with patients and their families via telehealth platforms. As a remote employee, you will engage with a multidisciplinary team composed of psychiatrists, psychologists, and counselors to coordinate comprehensive care.

Key Responsibilities
  • Conduct thorough psychological assessments of patients referred to hospice care, identifying their emotional and mental health needs.
  • Develop individualized treatment plans that address the unique concerns of patients and their families, focusing on crisis intervention and emotional support during difficult life transitions.
  • Provide high-quality individual, group, and family counseling, offering therapeutic techniques tailored to the needs and preferences of the client.
  • Monitor and evaluate client progress towards established treatment goals, ensuring each patient's journey is individualized and results-oriented.
  • Work closely with other healthcare professionals to ensure cohesive treatment and improvement in patient care outcomes, sharing insights from therapy sessions that contribute to holistic care.
  • Perform crisis intervention services for patients experiencing acute emotional distress, providing immediate and effective support.
  • Document all client interactions meticulously in our electronic health record system, ensuring compliance with legal and ethical standards.
  • Stay updated with the latest research and trends in clinical social work practices, attending professional workshops and training as needed.
Patient Population / Client Focus

Our ideal candidates will work with a diverse patient population, including individuals facing severe mental health challenges, end-of-life issues, and grief associated with loss. You will provide compassionate care to clients who may be experiencing anxiety, depression, and trauma related to their situation or that of a loved one. Your role will be critical in guiding families through the psychosocial challenges inherent in hospice care.

Qualifications
  • A Master's degree in Social Work (MSW) from an accredited institution is required.
  • A valid state license to practice as a Licensed Clinical Social Worker (LCSW) in Arizona.
  • Exceptional communication and interpersonal skills to effectively connect with a varied client demographic.
  • Proven ability to handle sensitive information with confidentiality and professionalism.
  • A talent for working autonomously as well as collaboratively in a remote team environment.
Preferred Experience
  • Prior experience in a hospice or palliative care setting strongly preferred.
  • Demonstrated expertise in conducting assessments and creating personalized treatment plans.
  • Familiarity with telehealth platforms and remote counseling practices.

The Transformative Psychotherapy Center offers a competitive salary range of USD 80,000 – 100,000 annually.
