Job description

Swope Health is seeking a compassionate and mission-driven Community Health worker to be a part of the Primary Care Home Health team of caregivers addressing social determinants of health, and focusing on reducing financial barriers that impact access to care. The Community Health Worker assists individuals and families in enrolling in public benefits such as Medicaid and the Affordable Care Act(ACA) marketplace, connecting them to critical resources, and providing financial literacy education. Community Health worker act as an advocate for the patient, acting a go-between between our providers and patients. 

Primary Responsibilities: 

• Conduct outreach and help enrollees engage in preventive care, chronic disease management, and
recommended services (Care Coordination; Individual and Family Support).
• Participate in care-team planning, huddles, case review, and information exchange (Care
Coordination).
• Provide education on health conditions, treatment plans, self-management, and home monitoring
(Health Promotion).
• Identify and connect enrollees with community resources and drivers-of-health supports (Referral to
Community and Social Support Services).
• Assist with clinical and non-clinical referrals, tracking, and follow-up (Care Coordination).
• Support access to transportation, food, housing, personal care, equipment, and other enabling
services (Referral to Community and Social Support Services).
• Co-lead health education groups and wellness or prevention initiatives (Health Promotion).
• Support medication and treatment adherence (Health Promotion; Comprehensive Transitional Care).
• Facilitate goal setting and help problem-solve barriers to the care plan (Comprehensive Care
Management; Health Promotion)

Successful Candidate will have:

  • High school diploma or GED equivalent.
  • Bilingual (English/Spanish) 
  • Certification or formal training as a Community Health worker highly preferred.
  • Certified applications counselor (CAC) certification highly preferred.  
  • Patience and ability to establish rapport with this population.
  • Flexible schedule and transportation.
  • Knowledgeable about diverse clientele.
  • Ability to write clearly and distinctly.
