Job description

Swope Health is seeking a compassionate and mission-driven Intake Clinician to support our Therapy team in providing the highest quality care for the people in our community who need it the most. The Intake Clinician is responsible for completing initial psychosocial assessments and diagnoses for children and adults with serious emotional disturbances, serious mental illness, and substance use disorders (SUD), in order to identify and refer to appropriate Swope Health Behavioral Health (BH) programs, SUD programs and Medical Clinics. Primary Responsibilities:
  • Facilitate an assessment interview that produces sufficient information to identify a BH and or SUD diagnosis, if such is present.
  • Ensure timely completion of initial psycho-social assessments for children, youth, and adults requesting BH and/or SUD services that integrate physical and mental health concerns and identify strengths and functional challenges.
  • Assess for risk of harm to self or others and take appropriate action to ensure safety.
  • Make immediate referral, per protocol(s), for Swope Health BH and SUD services appropriate to support identified challenges.
  • Facilitate referral to Swope Health Medical services to address any medical challenges identified.
  • Facilitate referral to other community resources necessary to address identified challenges
  • Collaborate with other providers and/or programs to best meet client needs.
  • Complete required documentation according to established standards and time frames.
  • Maintain productivity standards as established by Swope Health.
  • Participate in Performance Improvement activities related to Swope Health Quality Improvement Initiatives.
  • Attend and participate in Department meetings as appropriate.
  • Must complete annual agency trainings as required by Swope Health and participate in ongoing trainings to meet continued education licensure requirements and department needs.
  • Attend ADA department and Swope Health Services required meetings.
  • Other duties as assigned.

Education/Qualification
  • Master's Degree in either Psychology, Social Work, or Counseling and a license in the State of Missouri. Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW)
  • Minimum of three (3) years or professional working with serious and persistent mental illness (SPMI) with adults and
  • Proficient knowledge of health and human services and community resources.
    serious emotional disturbance (SED) with children. Preferably, with one year in a community mental health setting.
  • Applicants should be comfortable with providing telehealth services.
  • Working knowledge of the DSM- 5 and the ability to assess mental health disorders
