Work Environment

This position is fully remote, allowing for flexibility in both work hours and location. While you will be working independently from your home office, you will remain fully integrated with our interdisciplinary team through virtual meetings, collaborative case discussions, and continued training sessions. Key Responsibilities

• Conduct ongoing assessments to determine and address the mental health needs of patients, ensuring that the care provided is responsive to individual circumstances.
• Maintain open communication with supervisors, direct care staff, family members, and other stakeholders about patients' progress and treatment plans.
• Coordinate and develop a schedule of appointments, home visits, and necessary observations for effective monitoring of patient progress.
• Lead home visits to conduct assessments, collect pertinent data, and engage in meaningful interactions that support patient recovery.
• Implement and monitor the effectiveness of individualized program plans, ensuring that therapeutic interventions align with established goals.
• Participate in Interdisciplinary Team Meetings to discuss treatment strategies and contribute to the continuity of care.
• Document monthly and quarterly progress notes accurately and timely, reflecting the evolution of patient care.
• Organize and provide training to staff members, ensuring adherence to documentation standards and enhancing team competencies.
• Offer follow-up interviews and observations to assess ongoing needs and adjust treatment plans as necessary.
• Act as a liaison between the organization, the public, and the mental health community to advocate for best practices and patient care improvements.

Patient Population / Client Focus

Your expertise as an LPC will be utilized to serve a broad range of clients, including both adults and youth struggling with mental health issues. Your work will center around effective communication, trust-building, and evidence-based therapeutic practices, targeting issues such as anxiety, depression, trauma, and substance misuse. You will assist clients from various backgrounds and experiences, contributing to their overall mental wellness journey.

Qualifications

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
• An active and unrestricted Professional Counselor (LPC) licensure in the state of Texas.
• A minimum of 2 years of experience in a clinical counseling setting, demonstrating your proficiency in therapeutic methodologies.
• Specialization in areas such as family therapy, couples therapy, trauma, or substance misuse is preferred.
• A solid understanding of relevant legal and ethical standards in the counseling field.
• Excellent interpersonal and communication skills, essential for building rapport with a diverse clientele.
• Proven ability to collaborate effectively within a healthcare team environment.
• Proficiency in using Electronic Medical Records (EMR) software and other relevant technologies.

Preferred Experience

Applicants with experience in specialized counseling areas such as trauma-informed care or substance-related disorders will be prioritized, as these skills can significantly enhance our outreach and effectiveness in serving clients. Familiarity with therapeutic models such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) will also be seen as an asset.
