Key Responsibilities
  • Conduct ongoing assessments to analyze the needs of the diverse clientele we serve, adjusting treatment plans as necessary to ensure effective care.
  • Communicate regularly with supervisors, direct care staff, family members, and guardians to keep all parties informed about the client’s treatment progress and goals.
  • Schedule appointments and home visits, ensuring clear documentation and communication of work schedules to enhance client engagement and support.
  • Coordinate and perform home visits to conduct in-depth observations and assessments, including chart reviews and interviews, to better understand the client’s environment and support requirements.
  • Implement and monitor adherence to individualized program plans, ensuring clients receive the interventions and care needed for their progression.
  • Participate actively in Interdisciplinary Team Meetings, contributing insights and collaborative strategies to optimize client care.
  • Complete detailed monthly and quarterly progress notes that highlight client advancements and outline any adjustments to treatment plans.
  • Facilitate training sessions for staff and team members, ensuring that everyone is well-prepared to implement program plans and maintain accurate documentation.
  • Provide ongoing follow-up interviews and observations to address the evolving needs of clients, offering support and accountability.
  • Maintain organized and accessible records, ensuring that critical information is readily available for both internal use and collaboration with external partners.
  • Act as a liaison with the broader public and local mental health community, striving to build resources and support systems that align with client needs.
  • Engage in the development of training materials aimed at staff enhancement, ultimately improving program effectiveness for individuals.
Patient Population / Client Focus

You will work with a diverse client population that includes both adults and youth, necessitating a sensitive and tailored approach to each individual's unique circumstances. Our clinic serves various backgrounds, providing you with the chance to implement multifaceted strategies and interventions for mental health issues including trauma, family dynamics, and substance misuse.

Qualifications
  • A Master’s or Doctoral degree in Counseling, Psychology, or a related field is essential for this position.
  • Must hold an active and unrestricted Professional Counselor (LPC) license within the state of Colorado.
  • At least 2 years of counseling experience in a clinical context is required, with a focus on practical and applied methodologies.
  • A background or specialization in family therapy, couples therapy, trauma-informed care, or substance misuse is highly desirable.
  • Strong understanding of legal and ethical standards in mental health practice, ensuring compliance and best-practice care.
  • Demonstrated ability to build rapport with clients from varied backgrounds and establish meaningful therapeutic relationships.
  • Proficiency in collaboration as part of a healthcare team, showcasing effective communication and cooperative skills.
  • Experience with electronic medical record (EMR) systems and other related technologies preferred.
Preferred Experience
  • Previous work in community health settings or similar outpatient clinics, with a focus on accessible mental health services.
  • Experience in utilizing evidence-based treatment modalities and therapies, particularly those addressing trauma and substance abuse.
