Work Environment

Key Responsibilities

As a Physical Therapist at Mindful Physical Therapy, your responsibilities will include:

• Conducting thorough patient evaluations to assess physical function, mobility, and activity limitations.
• Developing detailed, individualized treatment plans based on patient assessments and best practice methodologies.
• Implementing therapeutic interventions and monitoring progress through regular evaluations and adjustments to treatment plans.
• Educating patients and families on exercises, modalities, and self-management techniques to support ongoing recovery.
• Collaborating with physicians and interdisciplinary team members to coordinate patient care, ensuring continuity and quality throughout recovery.
• Maintaining accurate documentation of patient treatments, progress notes, and discharge plans in adherence to all regulatory and professional standards.
• Participating in community outreach initiatives and staff training to promote health awareness and physical therapy practices.

Patient Population / Client Focus

This role will primarily serve a diverse patient population comprising individuals recovering from surgeries, managing chronic conditions, or rehabilitating from injuries. You will work closely with patients across different age groups and cultural backgrounds, ensuring that every individual receives compassionate care tailored to their specific circumstances. Your approach will include understanding the unique challenges each patient faces and addressing them with empathy and respect.

Qualifications

To excel in this role, candidates must meet the following qualifications:

• Bachelor’s Degree in Physical Therapy from an accredited program.
• Active State License in Physical Therapy and Basic Life Support (BLS) certification.
• Strong written and verbal communication skills necessary for effective patient interaction and documentation.
• Proficiency in using electronic medical record systems for documentation and communication.
• Commitment to maintaining licensure and staying updated with continuing education in the field.

Preferred Experience

We prefer candidates who have a minimum of two years of clinical experience in a rehabilitation setting. Compensation and Benefits
