Work Environment

This role operates entirely in a remote work setting, allowing for flexibility and work-life balance. As part of our hospice care team, you will connect with clients and provide therapeutic support via telephone or virtual platforms. Key Responsibilities

As an LCSW at Metrapolis Behavioral Center, you will take on a variety of responsibilities that directly influence the well-being of our patients:

  • Conduct thorough assessments of clients’ counseling needs and substance use issues to inform tailored treatment plans.
  • Utilize the DSM criteria to evaluate clients for substance abuse diagnoses, ensuring proper care levels are determined in alignment with ASAM guidelines.
  • Prepare and oversee the development of the substance abuse counseling component within client treatment plans, focusing on individualized care pathways.
  • Design comprehensive discharge and continuum of care plans, facilitating clients' transitions to the appropriate levels of support following their hospice care.
  • Maintain meticulous documentation of all client interactions, including counseling services, assessments, referrals, and follow-up activities, ensuring clinical records reflect the full scope of care delivered.
  • Manage an assigned client caseload, overseeing ongoing treatment planning, clinical charting, and regular updates for case management and utilization review needs.
  • Coordinate necessary support services for clients and their families while recording all provided services in clinical records for accountability and continuity.
  • Maintain active communication with referral sources—such as legal representatives, educational institutions, and employers—to ensure comprehensive support for clients.
  • Work collaboratively with a multidisciplinary team to facilitate crisis interventions tailored to assigned clients in need.
Patient Population / Client Focus

You will primarily serve patients in hospice care, many of whom may be grappling with complex emotional and psychological challenges related to terminal illness and substance use disorders. Your role will involve not only addressing these issues but also promoting an environment conducive to long-term recovery and emotional healing.

Qualifications

The ideal candidate will possess the following qualifications:

  • A valid LCSW license in the state of Oregon is required.
  • A Bachelor's degree in Social Work or a related field; advanced degrees are preferred but not mandatory.
  • Proven professionalism with a strong sense of therapeutic boundaries is essential, along with excellent cultural competency and self-management skills.
  • Demonstrated organizational capabilities and effective time management skills are crucial for success in this role.
Preferred Experience

Experience in hospice care or with co-occurring mental health and substance use disorders is highly desirable. Familiarity with treatment methodologies specific to addiction recovery will greatly benefit your effectiveness in this role.

Compensation and Benefits

The role offers a competitive salary range of $70,000 to $100,000 annually, commensurate with experience and qualifications. In addition to salary, we provide a comprehensive benefits package, including:
