Job description

Fraser is hiring a Group Home Coordinator / Designated House Coordinator (DHC) to join our Community Living team. In this role, you will guide and train staff, ensure compliance with Fraser policies and 245D regulations, and support continuous improvement in services. You will work directly with the individuals we serve, helping them achieve their personal goals, maintain independence, and receive assistance with daily living activities and personal cares as needed. Responsibilities also include providing onsite staff training, monitoring service quality, and ensuring the home operates in alignment with Fraser's mission and standards. This is a hands-on leadership role, and a portion of your weekly schedule will include providing direct care and support to individuals receiving services.
Responsibilities:
  • Provide on-site supervision and leadership to staff at the group home or residential location.
  • Ensure compliance with 245D regulations and Fraser policies.
  • Support active treatment service development and implementation.
  • Lead continuous quality improvement initiatives at the location.
  • Supervise staff and support their professional development.
  • Serve as a primary point of contact for clients, families, and community partners.
  • Monitor and maintain accurate client and staff records, including documentation required by regulations.
  • Participate in emergency response and preparedness activities as needed.

Requirements:
Candidates must qualify based in one of the following three ways, as assigned by 245D, which includes a combination of work experience and education.
  • HS Diploma or Equivalent AND four (4) years of full-time work experience (8,000 hours) providing direct care services to persons with disabilities or nursing/assisted living home experience
  • BA/BS in Human Services or related field AND 1 year of full-time work experience providing direct care services to persons with disabilities
  • Associate degree in Human Services or related field AND 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities
