Empowerment Pathways invites applications for the position of Licensed Clinical Social Worker (LCSW). This full-time role is suited for compassionate, skilled social workers who are enthused about improving the lives of patients in a hospice setting. Work Environment

This position is remote, offering flexibility while you work from the comfort of your own home in Frisco, Texas. However, you will engage closely with both clients and the multidisciplinary team at our hospice center. The collaborative atmosphere emphasizes teamwork and shared goals, ensuring that every case is addressed with care, consistency, and professionalism.

Key Responsibilities

• Engage directly with a caseload of clients suffering from major mental health issues, conducting both individual and group therapy sessions.
• Work in collaboration with other healthcare professionals, ensuring a unified approach to patient care that meets the diverse needs of our client population.
• Implement evidence-based assessment tools to screen clients, helping determine the level of intervention required and crafting personalized treatment plans.
• Facilitate integrated care plans that coordinate efforts across various healthcare providers, maintaining open lines of communication with clients, their families, and external partners.
• Conduct thorough documentation, ensuring accurate reporting on assessments, treatment plans, and ongoing care progress.
• Provide crisis intervention services when needed, employing de-escalation techniques and promoting a safe environment for clients.
• Participate in quality improvement initiatives and committees aimed at enhancing service delivery and client satisfaction.
• Provide on-call support as designated, ready to address urgent client needs outside of regular office hours.

Patient Population / Client Focus

Your primary focus will be on a diverse client group within the hospice community, many of whom are grappling with terminal illnesses, chronic health conditions, and emotional distress. It is essential to possess a deep understanding of the complexities associated with end-of-life care and to provide not only emotional support but also practical resources tailored to the individual needs of clients and their families.

Qualifications

• A Master’s degree in Social Work (MSW) is required along with a valid LCSW license.
• Expert knowledge of mental health assessment, diagnosis, and appropriate treatment modalities relevant to the hospice setting.
• Strong verbal and written communication skills, as well as demonstrated organizational acumen.
• Ability to build positive rapport with clients and colleagues, establishing trust and respect.
• Cultural competency to effectively engage with individuals from a variety of backgrounds, traditions, and belief systems.

Preferred Experience

• A minimum of two years of clinical experience as a social worker, preferably in a hospice or healthcare setting.
• Familiarity with various therapeutic techniques and a strong understanding of the documentation required for compliance and quality assurance.
• Experience in crisis intervention and management, ensuring competent risk assessment and mitigation.

Compensation and Benefits

Empowerment Pathways offers a competitive salary range of USD 70,000 to 85,000 per year, commensurate with experience.
