Position Overview

Empowerment Pathways is seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote position. This role is pivotal for delivering compassionate care to individuals facing complex mental health challenges. As a social worker at our community health clinic, you will provide vital support to clients dealing with severe emotional disorders and major mental illnesses, helping them navigate their journeys toward recovery and empowerment.

The social worker will operate primarily in a remote setting, allowing for flexibility and efficient communication with clients and team members. Key Responsibilities

• Provide direct clinical services to clients via telehealth platforms, addressing mental health needs through assessment, diagnosis, and treatment.
• Develop and implement integrated care plans tailored to individual client needs, ensuring access to essential services and resources.
• Conduct brief, evidence-based screenings with clients, following up with detailed assessments when necessary to clarify diagnoses and treatment paths.
• Participate as an active team member, contributing to team meetings and collaborating on treatment planning to achieve common goals.
• Engage in crisis intervention efforts, providing timely support to clients experiencing acute mental health crises.
• Carry a caseload of clients that includes assessment and therapy sessions, both individual and group, ensuring that services are client-centered and evidence-based.
• Monitor client progress and outcomes, adjusting treatment plans as needed based on feedback and assessments.
• Ensure timely and accurate completion of documentation, reports, and data submissions related to client care.
• Participate in organizational committees aimed at enhancing service delivery and quality improvement initiatives.
• Provide on-call support during and after program hours as required, ensuring clients receive timely interventions.

Patient Population / Client Focus

This role focuses on serving individuals who experience significant mental health challenges, including major mental illnesses like depression, anxiety, and bipolar disorder, as well as severe emotional disorders. Our clients often come from underprivileged backgrounds, requiring a compassionate approach that acknowledges their unique circumstances and promotes empowerment. The work environment fosters diversity, encouraging cultural competency and understanding of various traditions and beliefs among clients.

Qualifications

• Master's degree in Social Work from an accredited institution, with a valid LCSW license in Oregon.
• Expertise in assessment, diagnosis, and treatment planning for mental health populations, as well as risk assessment and crisis management.
• Strong communication skills, both verbal and written, with a capacity to build rapport and establish trust with clients and colleagues.
• Demonstrated organizational skills, with an ability to manage multiple cases effectively in a busy clinical environment.
• Competency in addressing age-related and developmental needs of diverse populations.

Preferred Experience

• A minimum of 2 years of experience in a clinical social work setting, particularly with a focus on mental health.
• Familiarity with community health systems, particularly in underserved areas, is highly desirable.
• Experience with evidence-based therapeutic techniques and assessment tools.

Compensation and Benefits

The salary range for this position is between USD 70,000 and 85,000 annually, commensurate with experience and qualifications.
