BJC Medical Group is seeking a Board Certified/Board Eligible Internal Medicine or Geriatrics Physician to join West County Medical Associates in a Post-Acute Care role serving patients throughout the West St. Louis County region.

This position is ideal for a physician who enjoys managing medically complex adult and geriatric patients, collaborating across the continuum of care, and making a meaningful impact on patient outcomes during critical transitions from hospital to post-acute settings.


Practice Overview

The Post-Acute Care Physician will provide comprehensive medical care within skilled nursing facilities, rehabilitation centers, assisted living communities, and long-term care facilities. Working alongside an interdisciplinary care team, the physician will focus on improving quality outcomes, reducing readmissions, and ensuring seamless transitions of care.

Key Responsibilities
  • Provide admission, follow-up, and transitional care services in post-acute and long-term care settings.
  • Manage acute and chronic medical conditions in adult and geriatric populations.
  • Coordinate care transitions from Barnes-Jewish West County Hospital, Barnes-Jewish Hospital, and other referring facilities.
  • Collaborate with advanced practice providers, nursing staff, therapists, social workers, and specialists.
  • Participate in interdisciplinary care conferences and family meetings.
  • Conduct goals-of-care and advance care planning discussions.
  • Drive quality initiatives focused on reducing avoidable hospitalizations and improving patient outcomes.
  • Complete accurate and timely EMR documentation.
  • Foster strong relationships with facility partners and referral sources across the community.

QualificationsRequired
  • MD or DO from an accredited medical school.
  • Board Certified or Board Eligible in:
    • Internal Medicine
    • Geriatric Medicine
  • Eligible for Missouri medical licensure.
  • DEA certification.
  • Commitment to patient-centered, team-based care.
Preferred
  • Fellowship training in Geriatric Medicine.
  • Experience in Post-Acute Care, Skilled Nursing, Long-Term Care, Hospital Medicine, or Transitional Care.
  • Interest in value-based care and population health management.

  • Manages the medical care of patient panel by providing or otherwise arranging for inpatient hospital care of physician's patients, either through regular hospital rounds, making arrangements with one or more hospitalist(s) or other qualified physician to provide coverage for physician's hospitalized patients consistent with Medical Staff requirements.
  • Collaborates with patients, families, and members of the care team to ensure excellent patient care outcomes at the clinic location(s) designated by BJC and any other BJC clinical outreach location to which physician may be assigned as patient care demands.
  • Performs and documents medical histories and physicals in the patient's medical record as required by hospital medical staff bylaws.
  • Provides or arranges for call coverage for clinic patients and inpatient call coverage in a manner acceptable to BJC and in accordance with Medical Staff bylaws, while observing and following all BJC policies and procedures and all applicable legal, ethical and professional standards.
  • Collaborates and teaches advanced practice providers, support staff or any care team member assigned in the care of physician's patient panel.
  • BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
