Overview
We are seeking an experienced Nurse Practitioner to join a PACE (Program of All-Inclusive Care for the Elderly) team in Evansville, Indiana. This role is ideal for an NP who values continuity of care, relationship-based medicine, and a predictable weekday schedule.
Details
• Primary Care | Geriatric population (55+)
• Monday–Friday, 8:00am–4:30pm
• Full-time, onsite with occasional home/facility visits
• Average 5–6 patients per day
• Locums assignment (3–4 months) with strong extension potential
• Phone-only call, rotated with other providers
Requirements
• Active Indiana NP license
• Active DEA
• Board certified (AANP or ANCC)
• Primary care experience with adult/geriatric patients
• Experience managing complex chronic conditions
