This full-time remote position operates within a community health clinic framework, allowing for flexible work hours with occasional evening and weekend commitments based on client needs. You will collaborate with a passionate team of social workers, mental health professionals, and community partners who share a common goal of enhancing the well-being of individuals facing social adversities.

• Conduct thorough individualized assessments to identify clients' social, emotional, and environmental stressors, and develop tailored service plans that outline specific goals and interventions.
• Provide individual and family counseling, offering emotional support and practical strategies to help clients navigate life challenges, such as trauma, crisis situations, and familial conflicts.
• Implement crisis intervention strategies, perform risk assessments, and create safety plans when necessary, ensuring client safety and well-being.
• Facilitate client access to community resources and services, collaborating with local agencies and organizations to provide holistic support tailored to each client's unique situation.
• Engage actively in community outreach initiatives to promote awareness of agency services and expand the reach of support to underserved populations.
• Participate in interdisciplinary case conferences, provide case management oversight, and attend regular team meetings to foster collaboration and share insights for improved client outcomes.
• Maintain accurate and timely documentation of client interactions, assessments, and service plans in accordance with agency policies and state regulations.

This role requires a compassionate and empathetic approach when working with diverse client populations experiencing challenges such as homelessness, domestic violence, substance abuse, and poverty. Your expertise will help empower these individuals and families to overcome their barriers and access essential resources for healing and stability.

• A Master’s degree in Social Work (MSW) from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Strong understanding of social justice issues, community resources, and trauma-informed care principles.
• Exceptional communication skills, cultural competence, and a proactive problem-solving attitude are essential.

• Prior experience working in social service agencies, particularly within mental health or community health settings, is highly advantageous.
• Familiarity with evidence-based practices in counseling and case management will be beneficial.

We offer a competitive salary range of USD 60,000 to 100,000 annually, commensurate with experience and qualifications.
