Job description

Position Summary


The Clinical Coordinator will be a key contact for patients entering HMHโ€™s behavioral health programs, managing the pre-admission and admission process. This is a key new leadership role involving coordination with stakeholders encompassing initial consultations, intake assessments, liaison with divisional and agency leadership such as the Medical Director, and Program Directors, as well as community-based organizations and regulatory agencies to ensure timely and appropriate service referrals.

The Clinical Coordinator will conduct outreach and coordinate with external resources to enhance treatment access, particularly for Asian Youth & Young Adults (A&YAs) and their families. This includes providing one-on-one navigation support to both new and existing patients to ensure seamless intake appointments with program directors.


Key Responsibilities:

  • Coordinate all referrals for mental health services, including direct patient referrals, hospital referrals, internal referrals, and referrals from outside agencies.
  • Complete all pre-admission screenings and required documentation in alignment with agency protocols and regulatory standards.
  • Utilize evidence-based practices and interventions to screen all new patients and provide referrals as appropriate to internal and external services.
  • Partner with Program Directors to assign cases to therapists/counselors as indicated based on assessing clinical compatibility and patient needs.
  • Ensure that individuals referred to services are connected to appropriate programs and receive the necessary level of care, including follow-up when needed.
  • Collaborate with administrative staff to ensure initial appointments with providers are expedited and to promote engagement in the treatment process.
  • Establish and implement an ongoing tracking system to monitor and report on referrals, admissions, patient flow, and service capacities to ensure timely access to care and appropriate service delivery.
  • Create policies and procedures to address the needs of A&YAs (e.g. parental/guardian consent, intake forms).
  • Conduct community outreach and engagement to expand the agencyโ€™s referral network and strengthen partnerships with external providers.
  • Collaborate closely with the Program Directors, Medical Director, and the Assistant Executive Director to enhance the agencyโ€™s capacity to improve access to services.
  • Attend professional meetings, educational conferences, and workshops, to maintain and improve professional competence.
  • Actively participate in an interdisciplinary team.

Minimum Qualifications:

  • Master's degree in social work, Counseling, or Psychology, or related field
  • Prefer a valid license in New York - LMHC, LMSW, LCSW, PhD, PsyD, or licensed professional in a related field
  • Minimum of 1 year of experience working with individuals receiving mental health services
  • Bilingual fluency required, with demonstrated proficiency in Mandarin
  • Familiarity with admission criteria and regulatory guidelines set by the New York State Office of Mental Health (OMH) and the Office of Addiction Services and Support (OASAS).
  • Demonstrated ability to respond to crises independently and link individual to appropriate levels of care.
  • Strong organizational and communication skills with the ability to manage multiple referrals and prioritize tasks effectively.
  • Demonstrated cultural proficiency and experience working with communities of color and/or communities in urban, underserved neighborhoods that bear a disproportionately high burden of illness.
  • Understanding of racial/social justice and equity issues.
  • Proficiency in Microsoft Office Suite (Word, Excel, and PowerPoint) and electronic medical records, and willingness to learn new systems as needed.


Compensation Range: $52 - $58K per annum

Hours: Monday to Friday, 37.5 hours/week
