The program brings together a team of professionals, family members, and community resources to develop and implement a personalized plan of care. The goal is to ensure that all aspects of a person's life—emotional, social, educational, and physical—are addressed in a coordinated and holistic manner. Wraparound Programs are typically used for those involved in multiple service systems, such as mental health, child welfare, and juvenile justice. The approach emphasizes collaboration, family voice and choice, and building on individual strengths.
