• Conduct comprehensive psychosocial assessments by engaging with patients through interviews, keen observations, and meticulous review of medical records to understand the emotional, social, and environmental barriers impacting their recovery.
• Provide empathetic counseling services to patients and their families, focusing on emotional support during significant adjustment periods related to their health challenges.
• Participate in weekly interdisciplinary team meetings to develop individualized treatment plans tailored to address the unique psychosocial needs of each patient.
• Assist clients in navigating and accessing vital community resources, such as support groups for mental health, vocational rehabilitation programs, or housing assistance, ensuring their holistic well-being.
• Document and monitor patient progress, offering timely updates and insights to the rehabilitation team to enhance overall treatment efficacy.

You will work with a diverse mix of clients, including those recovering from physical injuries, chronic illnesses, and mental health disorders. Our patient population ranges from adolescents to older adults, requiring a tailored approach that respects their individual journeys. Supporting patients through challenging transitions, you will play a pivotal role in connecting them with essential services and empowering them with coping strategies to enhance their overall quality of life.

• A Master's degree in Social Work from an accredited program is mandatory.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.
• A strong foundation in various therapeutic methodologies to address emotional and psychological concerns.
• Proficient skills in assessment and treatment planning that caters to diverse populations and complex cases.
